2004年全国HIV抗体诊断试剂临床质量评估工作情况汇报

            按照卫生部“全国艾滋病检测工作规范”要求，中国疾病预防控制中心性病艾滋病预防控制中心于2004年7月26日-8月31日组织了本年度全国HIV抗体诊断试剂临床质量评估，现将评估情况汇报如下。
    一、参评实验室

            参加本次试剂评估的有贵州省CDC、四川省CDC、广西壮族自治区CDC、新疆维吾尔自治区CDC艾滋病确认实验室和中国CDC性病艾滋病预防控制中心参比实验室。试剂考评由所有参评实验室的技术人员在参比实验室共同完成。
    二、被评估试剂
    

        按照“2004年全国HIV抗体诊断试剂临床质量评估方案”要求，参评试剂由参比实验室从市场上随机抽取，共获得18个厂家的HIV抗体酶联法诊断试剂（以下简称酶联试剂），分别为生物梅里埃、生物梅里埃抗原抗体、MUREX、华美、中山、万泰、吉比爱、金豪、科华、新创、永华、丽珠、绿科、普生、高达、耀华、月亮湾（华康）和理利；10个厂家的HIV抗体快速诊断试剂（以下简称快速试剂），分别为科华、雅培、PA、EY、艾康、新创、万泰、麦美华、中新科炬和金豪。共28个产品、28个批号（18种酶联试剂，均为第三代，10种快速试剂）。其中国产试剂21种（15种为酶联试剂，6种为快速试剂），进口试剂7种（3种为酶联试剂，4种为快速试剂）。
    

        三、血清盘构成
    

        1、酶联试剂评估血清盘
    

        由285份样品构成，包括已知结果样品95份和未知结果样品190份。
    

        （1）95份已知结果样品来自：参比实验室，共50份，25份阳性，25份阴性，2004年初经美国加州CDC参比实验室检测结果与参比实验室一致。药品生物制品检定所，共45份，20份阴性、20份阳性，5份系列血清。
    

        （2）190份未知结果样品：由四个省级参评实验室采自经静脉吸毒人群，其中阳性样品的比例平均为19.47%。
    

        2、快速试剂评估血清盘
    

        由293份样品构成，包括已知结果样品103份和未知结果样品190份。
    

        （1）103份已知结果样品来自：参比实验室，共60份，25份阳性，35份阴性。药品生物制品检定所，共43份，20份阴性、20份阳性，3份系列血清。
    

        （2）190份未知结果样品：由四个省级参评实验室采自经静脉吸毒人群，其中阳性样品的比例平均为19.47%。
    

        四、检测结果
    

        本次试剂评估共获得检测有效数据8060个（酶联法5130个，快速法2930个）。
    

        五、评估结果
    

        1、评估试剂的敏感性
    

        参评的18种酶联试剂的敏感性在82.35%-100%之间，其中为100 %的有八种，95-98%的有七种，敏感性在95%以下的有3种。
    

        参评的10种快速试剂的敏感性在80.95%-97.62%之间，其中为97 %的有一种，95%的有四种，敏感性在95%以下的有5种。见表2
    

        2、评估试剂的特异性
    

        参评的18种酶联试剂的特异性在83.00%-99.50%之间，其中为99 %的有一种，95-98%的有九种，特异性在95%以下的有8种。见表1
    

        参评的10种快速试剂的特异性在96.65%-99.52%之间，其中为99 %的有二种，96-98%的有八种，特异性全部在95%以上。见表2
    

        3、评估试剂的功效率
    

        参评的18种酶联试剂的功效率在88.07%-98.60%之间，其中为98 %的有二种，95-97%的有七种，功效率在95%以下的有9种。见表1。
    

        参评的10种快速试剂的功效率在93.17%-98.29%之间，其中为98 %的有一种，95-97%的有五种，功效率在95%以下的有4种。见表2。
